Our Policies


Get the Asthma and Allergy Treatment You Need

All co-pays are to be paid prior to being seen by Dr. Witkowska. If a balance is still owed, it must be paid at the time of your next visit. We'll gladly provide you with a receipt that contains all of the necessary information required by your insurance plan. Our practice accepts most major insurance plans, but we require a referral from your physician prior to making an appointment for HMO-managed care plans. If you're sick and need to be seen quickly, we'll work with you to schedule a same-day appointment.


We Want to Improve Your Quality of Life

You're responsible for providing the Allergy, Asthma & Sinus Practice with the proper referral documentation from your general physician. We also ask that you bring your insurance card with you to each visit and to notify us of any changes in your contact information or insurance status. Please call our office 24 hours in advance if you're unable to make it to your appointment or if you'll be late and we'll try to reschedule you for a later date.

Let Us Help You With Your Prescription Refills

If you have any questions concerning your asthma or allergy treatment and medications, contact our office and we'll assist you. To request an existing prescription refill, please call your pharmacy, otherwise our office requires between a seven-day and 24-hour notice for any new prescription refills that you need. If you haven't been seen in six months or more, you'll need to schedule an appointment in order to obtain any new refills through us. In order to assist us, we request that you bring all your medications to every visit to our immunotherapy clinic.
